OCTM Awards:

Applications and Nominations for the Ohio Council of Teachers of Mathematics’ OUTSTANDING MATHEMATICS CLASSROOM TEACHER AWARDS FOR 2008 are now being accepted. 

An elementary and secondary winner will be selected from each of the OCTM eight districts.  In addition, an outstanding college educator (teacher education or mathematics) from across the state will be selected. 

Committees appointed by each district director make selections of district winners during February.  District Directors will review the applications of winners at the March Board meeting and balloting for statewide award winners will take place during April.  District winners, Principals and Superintendents will be notified prior to June 1. 

The award winners will be honored at the NCTM Regional Conference in Cleveland (Oct. 16-17, 2008) at a reception to be held at the Crowne Plaza Hotel the evening of Oct. 16, 2008.  At that time, all district winners will be recognized and statewide awards will also be presented.  The Myrtle Miller/ Marijane Werner Award is presented to an elementary district winner and a secondary district winner will receive the Buck Martin Award recognizing them as the outstanding teachers on a statewide level.  The Kenneth Cummins Award is for college level instructors. Previous district winners may re-apply for the  Miller/Werner or Martin Awards, but may not repeat as district winners.

District award winners receive:
1) A Plaque and recognition at the state conference.
2) Paid registration for the state conference.
3) Complimentary banquet ticket for winner and a guest.
4) Small expense check to help defray conference cost.
5) 1 year membership in OCTM for winner or a colleague.

Instructions:

To be considered for a Classroom Teacher Award, please send the following items to your OCTM District Director, postmarked no later than February 15 of the respective year. (NOTE: If you do not know who your District Director is, click HERE to find your district on the state map. By clicking on your county, you will find the name and contact information of your District Director.) College applications and questions should be mailed to Laura Anfang, District Director-At-Large.

1. Complete the application form found below. The form should be typed (10 point or larger) with standard one inch margins. You may submit electronically or complete on line, print and mail, but be sure your supporting letters are mailed. Please remember: quality, not quantity, of supporting evidence is the most important criteria in determining district winners.

2. ONE signed letter of support written by a colleague, supervisor, student, or parent. The letter should provide specific examples of your abilities as a mathematics instructor rather than broad generalities. Letters of support must be limited to one typewritten page (10 point or larger) with standard one inch margins.

Information must be sent to the applicant's District Director.  This information can be found at the end of the application. 

Please note: You do not have to be nominated in order to complete the application; you may go directly to the application and nomination process below.

Do not forget the one-page letter of recommendation that is required to support your application, and that the deadline for submission to the District Director is Feb. 15, 2008.

Recommended procedure for submitting these forms:

1. Save the appropriate form to your computer by right-clicking on the appropriate link above and then choosing Save Target As... (Internet Explorer) or Save Link As... (Netscape Navigator).

2. Complete the application on your computer using your word processor for a .doc file or Adobe Acrobat Reader for a .pdf file.  If you do not have Adobe Acrobat Reader, you can download it free from http://www.adobe.com (link will open in a new window).

3. Submit the completed application as an e-mail attachment OR print it and mail the printed copy to your OCTM District Director.

4. Don't forget the one-page letter of recommendation that is required to support your application.  

5. Get started NOW so that you can finish before the application deadline: Feb. 15, 2008 of the respective year.
